Often the first sign is difficulty in bringing up the right word.
You know the word you want to use, but it just doesn't come up right away.
You can notice that Nancy Pelosi is starting to grasp for the right word.
Next you lose your train of thought in mid sentence.
You know what you are going to say when you start the sentence, but other thoughts flood into your mind and mid-way through the sentence, the other thoughts interfere with your ability to complete your original thought.
You can see this starting to manifest in Joe Bidden.
