The biggest fallacy in most conspiracy theories is that they would require perfect humans when all humans are flawed. Human cant keep secrets, make mistakes, have shifting allegiances and self-interest and more.
To that point:
The Islamic State spy who betrayed al-Baghdadi was an insider motivated by revenge
